Healthcare Mobile App
Summary
This is a mobile app Proof of Concept for CVS Signify Health, created for a client pitch. The company's mission revolves around connecting technology and patients to facilitate in-home virtual engagements, aiming to deliver a more connected, intuitive, and effective care experience.
After becoming a patient, individuals will receive an alert signaling their access to the app. Through the app, patients can conveniently view details about their prescribed medications and obtain information about their primary care provider.
Primary Care Patient Journey
Patient is alerted by email or text message to schedule visits for preventative care. After opening the app, push notifications will remind them of due preventative screenings. The patient can conveniently book either a Home Care or Telehealth session at a time that suits them. The app seamlessly supports Telehealth visits as well.
Pharmaceutical Patient Journey
Following each doctor visit or medication update, the patient receives an email or text notification prompting them to review all prescribed medications in the app. From there, they can request refills directly to their pharmacy and make payments conveniently within the app.
